Complete the form and return it to "Simply Walk, The Owls Nest, Chatsworth Road, Rowsley, MATLOCK, DE4 2EH" with a cheque made payable to "Simply Walk" for the number of people in the party multiplied by £60.00. If the booking is for a holiday within 8 weeks, please make the cheque for the full amount of the holiday.
